The reduction of irregularities in the use of “process fmea”: a study for a cold stamping process

The Failure Mode and Effect Analysis (FMEA) is a requirement for ISO/TS 16949. However, researches show nonconformities in its use by automotive suppliers. The objective of this study is to identify these nonconformities, aiming to suggest methods for minimizing this problem. Therefore, questionnaires were applied in seven automotive suppliers and, for each one, ten FMEA`s were analysed. The research identified nonconformities in the use of FMEA for the stamping process and highlighted seven influencing factors. Based on this data, a strategy is suggested. A partially parameterised FMEA spreadsheet is suggested and tested in three suppliers, showing significant improvements.
